Zuckerberg San Francisco General Hospital (ZSFG) has received the 2024 Bernard J. Tyson National Award for Excellence in Pursuit of Healthcare Equity for its significant efforts to reduce healthcare disparities, particularly among African American patients with heart failure. Acknowledging that heart failure is a leading cause of hospitalization in this demographic, ZSFG implemented an artificial intelligence-based decision support tool within its electronic health record system and launched an addiction medicine/cardiology co-management clinic. These initiatives have successfully closed a 5.4% gap in readmission rates between Black/African American heart failure patients and the broader patient population.
