CHOP Aims to Provide Care Across Continuum With Digital Monitoring Model

The Children’s Hospital of Philadelphia (CHOP) has launched an innovative digital care transformation model that extends care beyond conventional telehealth by incorporating four pillars: care at home, ambulatory digital care, virtual inpatient care, and affiliate network digital services. This model includes diverse services such as remote patient monitoring (RPM) and virtual urgent care, with the RPM initiative set to benefit approximately 50,000 patients annually through programs like CATCH that monitor high-risk pediatric patients post-discharge. By focusing on early intervention and continuous support for various conditions, including chronic diseases, CHOP’s approach aims to improve patient outcomes while promoting operational sustainability in healthcare.
